PAR-Q Forms and Waivers

The important "stuff". We know hanging upside and cartwheeling is the best thing in the world, it's why we do what we do. However, even with awesome, experienced and highly qualified staff, things can still go wrong in session. We want you to know that yes OF COURSE its fun but this kind of activity can also be dangerous. These forms below highlight the risks and hazards that can come with exercise and why our top priority is always your safety. 

Please select the appropriate form to complete. Adult class attendees of Acrobatics and Circus, you need to complete both I'm afraid!

Acrobatics and Circus Waiver

This form highlights the risks and hazards that come with participating in Acrobatics, Dance and Circus. This form must be completed by all grown ups with the responsibility of care to young participants attending classes with us.

*****This is a new form implemented since 2025 our previous form was outdated and therefore all must complete it. Thank you!

Please don't let all the tick boxes scare you. We are all fully qualified and experienced working with a range of medical conditions, additional requirements and injuries. This doesn't mean you can't attend but you must fill one of these good guys in so we can keep you from doing any more damage to prevent you from exercising as you want to.

If you are experiencing any difficulties completing your form, downloading it, attaching it to an email please note, you can print your form out and bring it along to your session. Alternatively you can email info.controlandmove@gmail.com about your difficulties and we will happily ensure you have forms to complete before the session.
